To keep dental costs down while maintaining in a high level of safety and professional care, Dr. Kimes' office has established the following payment plans for patients. Fees may be paid as follows:
1. Payment, or payment at the time of treatment, is acceptable by cash, personal check, Visa, Master Card, Discover or American Express.
2. Insurance: The staff will complete your insurance forms and file for any eligible benefits. However, all co-pays and non-covered services will be due on the day of service.
3. All accounts within 30 days are current. A finance charge of 1.5 percent per month will be assessed on all balances over 30 days.
